Servant House
Servant House is a 21-bed transitional housing program in which individuals may reside for up to 24 months. Servant House targets homeless disabled men who have a reasonable expectation to live independently and transition to permanent housing. Servant House provides housing, case management services, limited medical care, food, transportation, recreation, life skills, etc. Servant House has a preference for veterans, however we will take non-veterans if there is an available bed and there is not a veteran on the waiting list.
Servant House currently has openings and staff are accepting referrals.
To be eligible to live at Servant House individuals must be…..
I. Homeless (according to HUD’s definition): A person is considered homeless only when he/she resides in one of the places described below:
- In places not meant for human habitation, such as cars, parks, sidewalks, abandoned buildings (on the street).
- In an emergency shelter.
- In transitional or supportive housing for homeless persons who originally came from the streets or emergency shelters.
- In any of the above places but is spending a short time (up to 30 consecutive days) in a hospital or other institution.
- Is being evicted within a week from a private dwelling unit and no subsequent residence has been identified and lacks resources and support networks needed to obtain housing.
- Is being discharged within a week from an institution, such as a mental health or substance abuse treatment facility or a jail/prison, in which the person has been a resident for more than 30 consecutive days and no subsequent residence has been identified and the person lacks the resources and support networks needed to obtain housing.
- For example, a person being discharged from prison after more than 30 days is eligible ONLY IF no subsequent residence has been identified and the person does not have money, family or friends to provide housing.
- Is fleeing a domestic violence housing situation and no subsequent residence has been identified and lacks the resources and support networks needed to obtain housing.
II. Disabled: Federal laws define a person with a disability as “Any person who has a physical or mental impairment that substantially limits one or more major life activities; has a record of such impairment; or is regarded as having such an impairment.” Documentation must be provided substantiating a disability.
III. Male, age 18 or older
IV. Ambulatory
V. Clean for 30 days or more – if the client has a history of substance abuse, we must have documentation that the individual has been clean/sober for 30 days or more.
VI. Referred by an agency such as the VA, local nonprofit, hospital or other community agency. We do not take self-referrals. The referral form can be found below- please fax it to Emily Holmes at The Servant Center (336) 370-1540:
Referral Form

- Current medical records – medical physical within the last two years and recent mental health evaluation
- Current PPD – Updated tuberculosis test or negative chest x-ray within the last year
- Veterans DD214 – Copy of military discharge papers for all veterans
Servant House does NOT take individuals who:
- Are chronically physically or mentally ill and do not have a reasonable expectation to become independent and/or are sick enough to need a hospital, nursing care, rest home or assisted living (this is determined by the Servant House nursing staff).
- Are incontinent
- Are on Methadone or who require detoxification.
- Are taking narcotics
